Community Homes for Opportunity (Transitioned from Homes for Special Care)
Community Homes for Opportunity (CHO) is a recovery-oriented, person-centred supportive housing program for people with serious mental illness who may require 24-hour supportive care. The program is intended to assist tenants by providing appropriate housing and support services to achieve and maintain housing stability that is safe and affordable.

CMHA-Simcoe County is administering the CHO Program in Simcoe County. Blue Mountain Residence (Collingwood), Jennings Lodge (Penetanguishene), Residence on Water Street (Penetanguishene) and Residence of Lafontaine (Tiny) are now supported by CHO/CMHA-Simcoe County.

CMHA-Muskoka Parry Sound is administering the Community Homes for Opportunity (CHO) Program in the Muskoka area. Doe Lake (Gravenhurst), Lang Residence (Gravenhurst), Muskoka North Residential (Gravenhurst) and Manitoba Place (Bracebridge) are now supported by CHO/CMHA-Muskoka Parry Sound. Coordinated Access (CA) System - County of Simcoe
CA (Coordinated Access) is a process, not a program. A CA system is one by which individuals and families who are experiencing homelessness or at risk of homelessness are directed to access points in the community. At these access points, trained workers will use a common approach (including common intake forms and assessment tools) to assess the depth of need and help them complete the process that will prioritize them for housing support services. Clients will be matched to housing focused interventions in the community.
