How do I obtain a copy of a health record of a deceased patient?

Provide proof of legal authority with a written request to the Health Information Management Department outlining the records required. You must provide either:
  * a copy of the deceased person’s will naming you as the executor; or
  * in the absence of a will, “Certificate of Appointment of Estate Trustee” signed by a registrar of the court
    which can be obtained from the Estate Registrar Office, as follows:
