Mobile Treatment and Support Team 

The Mobile Treatment and Support Team (MTST) is a specialty transitional service that provides ongoing assessment and treatment for individuals that have recently been discharged from hospital until appropriate community resources are located that provide the level of ongoing support required. MTST is able to meet with individuals in our office and/or in the community, as required based on the needs of each individual and the impact of their illness. MTST also supports individuals who may be ordered for treatment by the Ontario Review Board and require follow-up in the community. 

The hours of operation are Monday to Friday from 8 a.m. to 4 p.m. and we are located at 1-952 Jones Road, Midland. 

Program Eligibility
Individuals 16 years and older who have recently been discharged from a tertiary mental health inpatient unit that have a serious mental illness (e.g., Schizophrenia, Schizoaffective Disorder, and/or Bipolar Disorder) and have complex care needs that require intensive support in the community. 

Referrals
Referrals to MTST must be completed by a Physician from a tertiary mental health inpatient unit using the Waypoint Referral Form. Completed referral forms need to be submitted to Waypoint’s Central Intake department. Access to MTST
Following acceptance to the MTST program, a team member will contact the client directly to begin services. The MTST team consists of psychiatrists, nurses, occupational therapists, social workers, and community support workers. The main goal of involvement with MTST is to maintain health and wellness, promote community re-integration, and minimize the likelihood of re-hospitalization. Access to other care providers is available as needed, including dieticians, spiritual care providers, and pharmacists.
