Planning a new Ontario Health Team to care for specialized populations
Collaborating with other agencies to ensure patients are receiving the best care available has always been a goal at Waypoint. Under the new Central Ontario Regional Ontario Health Team for Specialized Populations, this will continue.
Waypoint is partnering with ten agencies to provide specialized person-centred care for vulnerable people with highly complex needs, and their families and caregivers.
Deemed an innovative model under the Ontario government’s new Ontario Health Team (OHT) plan for health care system transformation, the partners will build on their specialized services with a focus on trauma support, mental health and addictions including children and youth, seniors health services and end of life care. The priority is to move forward on building capacity for child and youth mental health and addiction, expanding capacity for care of frail seniors, and providing culturally safe services for Indigenous peoples.
